Teaching video NeuroImage: isolated medial rectus palsy in midbrain infarction.

چکیده
A 76-year-old man with hypertension suddenly developed horizontal diplopia. Examination disclosed adduction deficit of left eye without dissociated abducting nystagmus and ocular tilt reaction (OTR) (video). The impaired adduction was similar for saccades, smooth pursuit, the vestibulo-ocular reflex, and convergence. Vertical eye movements were normal. Diffusion-weighted MRI demonstrated a circumscribed acute infarction in the left paramedian midbrain (figure). Isolated medial rectus palsy due to midbrain lesion is extremely rare1 and should be differentiated from internuclear ophthalmoplegia (INO). Impaired convergence, absent abducting nystagmus, and OTR in our patient support medial rectus palsy rather than INO.

متن کاملIsolated Medial Rectus Nuclear Palsy as a Rare Presentation of Midbrain Infarction
BACKGROUND Diplopia is a common subjective complaint that can be the first manifestation of a serious pathology. Here, we report a rare case of midbrain infarction involving the lateral subnucleus of the oculomotor nuclear complex presenting as diplopia, with no other stroke manifestations. CASE REPORT An 83-year-old right-handed white man with past medical history of diabetes mellitus, hyper...

113 To the Editor: Ophthalmoplegia can cause diplopia, with a lot of etiologies including cerebrovascular disease. Nuclear ophthalmoplegia resulted from brainstem infarction is often accompanied by other symptoms of brainstem dysfunction. It is fairly rare that midbrain infarction only manifests as isolated medial rectus nuclear palsy. Here, we provided such an uncommon case and discussed the p...
متن کاملIsolated superior rectus palsy due to contralateral midbrain infarction.
BACKGROUND Isolated superior rectus palsy due to a contralateral midbrain lesion has not been reported. CASE DESCRIPTION A 71-year-old woman suddenly developed diplopia. Examination showed that she had isolated superior rectus paresis. Magnetic resonance imaging showed a tiny infarct at the area of the oculomotor nucleus on the contralateral side. CONCLUSION Isolated superior rectus palsy m...

J.S. Kim, MD S.-H. Jeong, MD Y.-M. Oh, MD Y. Soon Yang, MD S.Y. Kim, MD A 78-year-old man with hypertension and diabetes was referred because of 3 days of diplopia and ophthalmoplegia. Neurologic examination disclosed exotropia of both eyes (wall-eyed) and bilateral internuclear ophthalmoplegia (WEBINO) with impaired convergence (figure, A).1 Vertical saccades and smooth pursuit were also limit...
